Second Passports

  1. Fill out Online DS-82 Application.
  2. Current passport with two years validity.
  3. Letter to the U.S. passport agency explaining the need of a second U.S. passport.
  4. Evidence of Immediate travel (If expedite process is required). Following two copies are required:
    • Travel itinerary indicating reserved or confirmed.
    • Copy of round trip ticket indicating travel to a foreign country.
  5. Two recent passport 2 x 2 inch photographs taken with in six months (Photograph quality requirements)
  6. Government Passport Fees, following fees are applicable.
  7. Authorization Letters (Download here) and (Download here) (that allows us to submit and receive your passport/application) Note: Please leave "Company Courier Name" blank.
